Today, Cambridgeshire and Peterborough Safeguarding Adult Partnership will publish a Safeguarding Adult Review into the tragic death of Miss Y. Our deepest sympathy continues to be extended to the family and friends of Miss Y. This review explores Miss Y’s story and the services that she received over a number of years.
At the heart of this Safeguarding Adult Review is a young woman who suffered from a range of mental health issues and the response and service she received from partner agencies. As part of the review process we had the opportunity to engage and speak with Miss Y’s family. We realise that this must have been extremely difficult for them and we are grateful that they shared their stories and experiences of Miss Y and helped to ensure Miss Y’s voice was heard through the report.
The involvement of multiple agencies in the lives of Miss Y and her family have been detailed, appraised and recommendations have been made. The partnership remains deeply committed to ensuring that agencies make the further changes that are needed to take forward the learning that has arisen from this Safeguarding Adult Review and Miss Y’s tragic death.
